Please complete the following forms prior to an evaluation with your supervising director. Give these forms to your director.

Intern Evaluations
The purpose of intern evaluations is to show the growth of the intern within the internship program. They help to identify strengths and weaknesses within the intern and the intern's work. Interns, specifically Drake students, will use these evaluations for their own academic benefit. Before each evaluation, supervisors should look at program specific expectations. These can be found on program specific research pages and the Staff and Intern Expectations page. Whenever possible supervisors should refer to these qualitative and quantitative expectations in the evaluation.A supervisor should fill out the evaluation form 1-2 days before meeting with the intern. This will allow the Internship Coordinator and Research Directors to contribute any praise or criticism to the evaluation. After completing the form, Google automatically generates an email to the account which filled out the form. This email contains the answers to the questions from the form and should be printed for supervisor's and intern's files.
Beginning Evaluation

Within this evaluation, supervisors need to be addressing problems that they have seen with both training and professional behavior. It is easier to correct mistakes early than to relearn how to do things. This is the perfect time to create goals for the intern to accomplish before the next evaluation and the rest of the internship.
Midterm Evaluation
https://forms.gle/KudBiEnJ49M2pCxQ8The midterm evaluation is conducted in the middle of the intern's time with Vote Smart. For a typical 130 hour intern, this happens after 65 hours completed.
If there are any problems or issues that have not been addressed by this time, it is crucial that you address them with this evaluation. The intern will be made aware in writing and verbally of any problems or improvements to make - this will provide a situation where there is no confusion about their quality of work in the office. This does not have to be a long meeting at all, just make sure to bring up anything that needs to be addressed. At this time, interns also complete a supervisor evaluation.
Final Evaluation Week
https://forms.gle/1rBhtzakktqxBBHu5This evaluation is done on the final day of the intern's time with Vote Smart.
The contents of this evaluation should include a review of the entire internship. This final evaluation may be used by the intern for final projects or other academic credit. Supervisors should make sure they are giving the intern a quality evaluation of substance. Interns complete a supervisor and program evaluation on their last day.

Intern Evaluation of Supervisors
During the intern's final day, they should be given the exit evaluation to fill out by the Internship Coordinator. The purpose of the exit eval is to allow the intern to give us feedback as an organization and as supervisors. The Exit evaluation needs to be filled out privately by the intern on their last day in the office. Staff members may NOT read exit evaluations unless given prior approval by the Internship Coordinator or National Director. When they have completed the evaluation, interns should be instructed to turn their evaluation directly in to the Internship Coordinator.| File | Last modified | Size |
