Policies:
Time off policies are set by the Staff Policies and Procedures Manual. Submit a Time Off Request form in advance. Note that any earned Paid Time Off will be applied by default when time off is taken.

Payroll Changes

If you notice an error in your paycheck or pay stub please contact the Office Manager.

To change the address on your pay stub or the formula for tax deductions, please submit a revised W4 to the Office Manager (found on the IRS website).

To set up direct deposit of your paycheck or change your bank account information, please submit a revised Direct Deposit form to the Office Manager.
